- Contact information for the complaining party;
- A statement confirming the belief that material use is unauthorized;
- A statement confirming the accuracy of the claim under penalty of perjury.

If you receive a takedown notice, you can counter with a written notification including your signature, details of the material, and a statement affirming your belief that removal was a mistake.
